Don’t go back more than three years. Be prepared to show that you’ve fully recovered. The word ever on an app is just too broad. You can narrow it to a reasonable time. If the employer discovers the injury and you’re well now, you’re probably also fine.
Absenteeism
Fewer than 10 days per year over a two-year period isn’t considered excessive. It gets an:
“Occasionally absent for minor illnesses.”
More, and you’ll be asked about it during the interview.
Military Status
Since dishonorable discharges are so rarely used, you should have no problem.
Conviction of Felonies
Answer “No” if you were under 18.

Use the full name of the company, abbreviating the address where necessary. If asked, its kind of business should be stated in a few words.

Don’t leave them blank. If you do, it looks like you’re trying to hide an employment gap.
